Khyber Pakhtunkhwa recorded 99 new dengue cases in the 24 hours to October 20, 2026, the highest single-day count the province has logged this year, taking its cumulative 2026 total to 4,091 confirmed infections. Kohat district, already the province's worst-affected area, accounted for more than a quarter of that total on its own.
Where the cases are concentrated
Of the provincial total, Kohat district alone had recorded 2,567 cases, according to data from the province's Integrated Disease Surveillance and Response System (IDSRS). Peshawar followed with 566 cases, Bannu with 192, Khyber district with 120 and Haripur with 112; every other district in the province was in single or low double digits. Of the 4,091 total, 144 patients were still being treated as of October 20 and 3,946 had recovered. The IDSRS also recorded a death linked to the outbreak in Dera Ismail Khan, of a resident of Tank district.
Kohat's October numbers show the outbreak accelerating rather than easing: the district added 80 new cases in the first twenty days of the month alone, compared with 24 in Bannu and 18 in Peshawar over the same period.
The provincial government's response
Two days before that record daily count, Governor Faisal Karim Kundi chaired a briefing in Peshawar on the outbreak, where officials put the provincial case count at 3,771 and the death toll at two. Advisor for Health Lieutenant General (retired) Nigar Johar and the provincial health secretary attended. Kundi is currently administering Khyber Pakhtunkhwa under the governor's rule and federal emergency imposed on the province in mid-October over a surge in militant violence, meaning the same office now has to run a dengue containment effort alongside the security measures that triggered the emergency in the first place.
At that briefing, officials said the province had prepared isolation wards with a combined capacity of 1,666 beds, and were running a public awareness campaign on prevention and control alongside an effort to strengthen the reporting and surveillance system used to detect new cases. The governor directed the Kohat district administration specifically to take "effective emergency measures" and report back on progress, and instructed other affected districts to coordinate immediate containment steps with their district administrations. Officials attributed the scale of the outbreak to a mix of factors, including climate conditions, unplanned urban growth, poor sanitation and the electricity load-shedding that leaves stored water sitting uncovered in many households — all conditions that favour the Aedes mosquito that spreads the virus.
How this compares with last year
Khyber Pakhtunkhwa's health department has said this year's case trajectory is still running below 2025, when the province logged more than 6,000 dengue cases for the full year, including over 1,000 in Charsadda alone after an outbreak across several of its union councils. We reported on dengue's spread across four provinces earlier this month, when Punjab, Sindh, Islamabad and Khyber Pakhtunkhwa were all logging several hundred new infections a day; Kohat's single-district total has since become one of the more concentrated hotspots in that broader outbreak.
What to watch
Dengue transmission typically continues through November before cooler temperatures suppress mosquito activity, and health officials involved in the Kohat response have warned that conditions over the coming weeks remain favourable for further breeding. Whether Kohat's case count keeps climbing at October's pace, or responds to the emergency measures the governor ordered on October 18, will determine whether the district's final tally approaches, or exceeds, the more than 1,000 cases Charsadda recorded in last year's worst-hit area. Travellers to Peshawar and surrounding districts can check current weather conditions for Peshawar, since the same humid, stagnant-water conditions driving the outbreak also shape the province's day-to-day forecast.
